Weak Bladder

I am at my wits end with my dog peeing on my carpet?
Let me begin with how much I love my Golden Cocker Retriever. She loves he tummy rubbed and loves me so much that no matter what she will not leave my side. Even when I am sleeping and my husband tries to lure her out with treats. ...Anyways...I have trained her rigorously, she is 2 years old and learns UNBELIEVABLY fast. Which is why I am at my wits end. Within a week I have flawlessly trained her to sit, stay, lay down, ring a bell to go outside, and go lay on the couch when she is being a bother or getting too hyper. I am in the process of training her recall. She is potty trained (by the book she is not bc she is peeing and pooping in the house all of the sudden) She has a huge back yard to herself. She is inside and getting love all day. She get walked and played with ALL OF THE TIME. She had a pooping problem in one of the bedrooms, and I kept that door shut and got her on a perfect poop schedule. 20 minutes after every meal like clockwork. So that should be good to go. NOW she is peeing 5 feet from the sliding door and on the carpet. The carpet is starting to look horrible bc of all the times I have cleaned it with professional pet cleaner. I know she does not have a weak bladder as she stays in bed with us with no accidents for 7-8 hours just fine. Our door is shut and I would smell if she messed in our bedroom. Trust me when I say....she goes out almost every hour....I used to have the bell up for her to go out, but then she started abusing it and ringing it for fun. So I watch her and as soon as she goes to the door, up i go to let her out. I stand there, point and day "go potty" and she does, then she is exuberantly praised and gets a treat RIGHT away. I do this everyday all day. There is not a time where she does not know that outside is where I am happy with her going, and I never let her go too long without pottying. I am not kidding, I do everything by the book. She is never out of my sight as she does not leave where she can see ME. She just plays with her raccoon toy, or chews on rawhides or is playing with me with the laser toy. Then out of no where, no sniffing or anything, as I look at her....before I can even run to her fast enough, she pees....on the carpet. I catch her right at the end, yell NO and put her outside and say "potty". I give her no reward or attention for about 5 minutes bc I know she will not remember what she did for long. I am at my wits end. I am going through a very very very hard time right now. I cannot mentally wrap my mind around this, and I cannot emotionally handle this anymore. I am on the verge of adopting her out, bc there is NOTHING more I can do. I try to crate train her, but she hurts herself in there, like damages her body trying to get out.

Did my bladder burst or ?
I fell like i pee a little on my self(Not a lot), i have always hated public restrooms so i would hold it since i can remember until recently that i have been going to pee when i feel like it which is often. I don't know if i have a UTI or a weak bladder but i'm worried that my bladder might have popped or something. What could be causing this small amount of urine to come out?? Please help =/ And has been going on over a month now

am i in my fertile days?? really wet underwear.....?
I was reading online that your discharge changes along with your cycle, it goes from dry (after your period), tack and then wet or slippery.. the other day it was tack, i did the finger test and it was just sticky and it was like that for 2 days then today im really wet, my underwear are as if i started to pee a little bit, when i look at them,its like half of the bit where u stick your pad on is wet, thats how bad it is! i dunno if i just have a weak bladder maybe? but me and my partner are not using any contraception, and were just avoiding fertile days to avoid pregnancy.. so could i be fertile?? my cycle averages on 46 days long, i got my period feb 4th and it ended on the 7th.. also, my cycles are all irregular because i came off the evra patch 8 months ago and since december my cycles have been 45-48 days
